Lilith, daughter of Mephisto and co-creator of Sanctuary, has been summoned back into the world. Her return tears at the fabric of the balance that has held for centuries, drawing heroes and demons alike toward a reckoning that will leave nothing unchanged. Six base-game classes — Barbarian, Sorcerer, Druid, Rogue, Necromancer, and with expansions Spiritborn, Paladin and Warlock — are each built on fundamentally different combat systems and playstyles.

Diablo IV is the fourth main entry in Blizzard's defining action RPG franchise, returning to the gothic horror atmosphere of Diablo II while building on the mechanical depth of Diablo III and adding a fully open world for the first time. Sanctuary is a contiguous, explorable landscape spanning five distinct regions — the frozen Fractured Peaks, the arid Dry Steppes, the corrupted Scosglen, the cursed Hawezar and the haunted Kehjistan — connected by roads, dungeons and open-world events. Loot acquisition and character building through the Paragon board system drive endgame progression alongside Nightmare Dungeons, Helltides, Pinnacle bosses and a seasonal live-service structure that adds new mechanics, story quests and cosmetics every three months. The first expansion, Vessel of Hatred (October 2024), added the Spiritborn class and the jungle region of Nahantu. The second, Lord of Hatred (April 2026), adds the Paladin, the Warlock and the Skovos Isles.
